Anatomical Studies on the Facial Canal and Facial Nerve in Koreans

Abstract
The facial nerve has one of great important function which is the expression of the feeling of the human being. So that facial paralysis produced by Bell¢¥s palsy, inflammatory invasion of the facial canal and nerve damage due to middle ear surgery may give the patient a permanent handicap in social life.
As tympanoplasty has recently increased, the possibility for facial nerve damage has also increased. Therefore correct diagnosis and successful surgery are depend upon detailed knowledge of the normal and aberrant facial canal and nerve.
The author carried out an anatomical study on the facial canal and facial nerve using 24 cases(male 21, female 3) in Koreans ranging from the age of 23 to the age of 79.
The results obtained were as follows:
1) The average length of each segments was as follow:
The labyrinthine segment was 9.92mm, the tympanic segment was 15.00mm and the vertical segment was 18. 50mm which extended one half the entire length.
2) The chorda tympani arose from the vertical segment at 8.48mm above the stylomastoid foramen, which was approximately half the vertical segment.
3) The angle formed by the vertical segment and the tympanic segment was 130150¡Æ in 20 cases (83.3%) of total cases.
4) The dimension of the facial nerve averaged 1.33 x 1.00mm--1. 13 x 0.84mm in the vertical segment and the dimension of the facial canal averaged 2. 77 x 1. 72mm-2.93 x 1. 78mm in the vertical segment.
5) The space in the facial canal occupied by the nerve ranged from 30. 0% to 53. 9% between the beginning of the canal and the region of pyramidal eminence, while it was approximately 20.0% from the region of pyramidal eminence to the stylomastoid foramen.
6) The wall of the facial canal on the side of the tympanic cavity was very thin at the horizontal segment, while it became thicker below the horizontal segment. On the contrary, the wall of the facial canal on the side of the mastoid antrum was very thick at the horizontal segment, while it became thinner at the vertical segment.
7) Of the blood vessels of the facial canal, the branch of the middle meningeal artery ran down along the side of the tympanic cavity, while the stylomastoid artery ascended along the side of the mastoid antrum. These two artery may have anastomosed at the region of the pyramidal eminence, where abundant blood vessels were seen.
8) The dehiscence of the facial canal wall was seen in 16 cases out of the 18 cases (88.9,¡Æx) and these defects ranged from 0.48 x 0. 18mm to 3.03 x 1.29mm.
9) The facial nerve was made up of several nerve bundles just below the geniculate ganglion but one nerve bundle was present in the distal portion of the ganglion.
